What to expect

In-Depth Onboarding Assessment

You will fill out an in-depth health questionnaire prior to your first appointment. Then meet in-office with Practitioner Lindsey for a comprehensive and educational based deep dive into your symptoms and health history. At the end of your visit, personalized recommendations for lifestyle changes, nutrition adjustments and recommended lab testing will be given.

Lab Testing Review

Your second visit will include an extensive review and discussion of your lab results. As well as a review of your lifestyle and nutrition implementation progress. A detailed personalized wellness plan based on your lab results will be given to you with actionable detailed directions.

Work Plan + Check-In

Your third visit will be a time to review your progress to date, overcome obstacles and barriers in your journey and make adjustments to your plan as needed.

Repeat Labs + Make Adjustments

Monitoring of labs and improvements will take place approx 3-4 months after initiating your wellness plan. This repeat lab visit will be a benchmark to assess your progress and improvements in health. Then Individual recommendations will be made after this visit for future appointment follow up timeline with the average patient having 5-7 visits with the provider within the first year and many graduating to check-in’s 1-2x per year.

  • New Patient Initial Consult: 75-100 minutes: $625

 

  • Report of Findings 2nd Visit Consult: $575

 

  • Follow up Consults 60-90 minutes for $315-$475.

 

-*Extra time spent will incur additional charges at hourly rates.

-Customized In-Depth Nutrition Plans are offered as add-on service for additional fee. 

-Forms, referrals, insurance requests/paperwork may be completed as additional services outside of the standard consult review for additional fee.

Testing is based on each individual case. With direct-to-patient discounted pricing 

  • 60+  Comprehensive Biomarkers Bloodwork Panel: An in-depth lab panel testing 5x more than a typical annual physical labs assesing your Metabolic, Liver, Kidney, Hormonal, Nutrient, Mineral, Inflammatory, Cardiovascular status
    • For non-insurance self-pay clients there is special pricing that has been negotiated on your behalf. Please note, the first lab panel is the most comprehensive and pricing ranging from $550 to $800. Please discuss discounted self-pay options with the office.
    • Insurance restricts what can be tested and covered. We work for you, not insurance companies.   It has been our experience insurances do not participate in covering advanced biomarkers testing finding root causes to your health concerns.

 

  • Comprehensive Hormone/Adrenal Cortisol: $175.00-275.00

 

  • Gut Health Specialty Microbiome  to assess digestion, absorption, microbiome $425-$475.

 

  • Toxin Testing: Heavy metals, Mold/Mycotoxins, Environmental chemicals and Toxins: $675-$875.

 

  • Comprehensive Food Sensitivity : $475 – $675.

Designed Health does not participate nor bill insurance companies.

The insurance model does not cover  Personalized Holistic – Performance Lifestyle  based Medicine. 

 Lindsey is considered an “out-of-network” provider.

You may try to submit receipts to your insurance. Please note we do not work with insurance and any insurance forms you request to support any potential coverage are subject to a fee.

Lindsey accepts debit, credit, HSA/FSA, personal check, or cash. Payment is due at time of service.

 

It is requested that you provide at least 48 hour notice of your intention to cancel or reschedule an appointment.

If you provide less notice than that or simply fail to keep your appointment, you will be charged for that appointment.
